Clinical Pharmacy Specialist - Palliative Care and Pain Medicine

REQUIREMENTS

Education: Pharm.D degree.

Preferred Education: None

Certification: All of the following:
Texas State Board of Pharmacy Pharmacist license within 90 days of job entry.

Preferred Certification: Board Certification in appropriate specialty area - Board of Pharm

Experience: Pharmacy Practice Residency (PGY 1) and Residency in the area of specialization (PGY 2).  Experience as a clinical pharmacist in the area of specialization may substitute for the required residency on a one to one basis. 

Preferred Experience: Experience as a clinical pharmacy specialist
